I've been there myself. When I burned out, it was a combinaton of my Chron's disease and stress (2008 financial crisis). Chron's went into overdrive and nearly killed me. I lost 14 kg in a few weeks, and I knew there was no fix for this.
I was told in no uncertain terms by doctors that it's over - my life, job, everythign I had built thus far. Took their advice and left it all, and went into healing mode for the next two years.
The good news is, they're wrong. Chron's is an autoimmune disease and untreatable by medical science, but not by you. I've been symptom free and perfectly healthy for about 14 years now, after sufferin like mad for previous 15 years. It can be done, but you're going to have to look inward for the solution and give it time.
I wrote a bit about my journey to healing here - if nothing else, I hope it gives you hope that you don't have to be stuck with the disease forever, even thought the first thing I would suggest is that you accept that possibility and make peace with it.

While I'm hoping the drugs I'm on help I know they might not and that it is very unlikely I'll be on them for more than a couple years so stories like yours are invaluable. Sorry to hear that. One of my aunts has Chrons Disease. It's rough but once you learn to manage it you will be back on your feet.
